You may arrange to have your official academic transcripts sent to another school or university by completing the Transcript Request Form and submitting the form along with $6 per request to the UW Colleges Registrar Office.

Transcript Requests

Official transcripts are issued by the Registrar's office at the UW Colleges Central Office in Madison. If you would like your official UW Colleges transcript sent to another institution, or if you would like a copy for yourself, the request must be submitted in writing (telephone requests are not accepted).

There is a $6 charge per transcript. All requests must include your signature to be valid. NOTE: If you check the "send now and after current term" box on the Transcript Request Form, that is considered two requests and therefore requires a $6 charge per request.

There are four ways to request an official transcript:

1. Via Mail/Form – download the Transcript Request Form, fill out form online, print form, sign form include check or credit card information and mail.
    
2. Via Fax – download the Transcript Request Form, fill out form online, print form, sign form, and fax form with credit card information.
    
3. Via Email – download the Transcript Request Form, fill out form, print form, sign form, scan form and email with credit card information.
    
4. Via Mail/Letter – to send a letter of request include your name, social security number, campus attended, dates of attendance, and name and address of the recipient. Sign your request letter, include check and mail.
