HOW TO OBTAIN YOUR PHOTO
ID CARD
1. IDENTIFICATION REQUIRED
Students, faculty, and staff who need a new or replacement
ID card must present some form of personal photo identification
such as a valid driver's license, passport, or state ID.
2. AUTHORIZATION
- Students: Newly admitted students may obtain
their initial card at no cost upon verification of enrollment
by the card office staff. Continuing or returning students
may obtain a card to replace a lost, stolen, or worn ID
as needed. Replacement cards may be subject to a fee.
- Faculty/Staff: The Photo ID Office can make
a faculty/staff ID card ONLY after the school has
entered the employee data into the appointment system. It
generally takes one business day for this authorization
to appear in the ID database. The initial card is issued
at no cost.
3. VALID ID CARDS
The ID card issued to an individual is valid throughout
a person's entire academic or employment career
at the University of Wisconsin
Baraboo\Sauk County. This card should be carried at
all times and presented upon request. Only the most recent
card issued to you is valid. If a card has been replaced and
an old ID is found and used, the card will be confiscated.
4. LOST AND FOUND CARDS
Any ID card that is found should be immediately forwarded
to the Business
Office . Anyone looking for a lost card can call 608-355-5210
to see if the card has been turned in.
5. OBTAINING A REPLACEMENT ID CARD
A non-refundable fee is charged to replace a lost or stolen
card. Cards that become worn or damaged may be surrendered
to the Business
Office and a new card will be issued. Worn cards may
be replaced at no charge if the card office determines the
damage to the ID is from normal wear. Fees are subject to

6. PROPERTY OF UW-Baraboo\Sauk
County
The Photo ID card is the property of UW-Baraboo\Sauk
County , is non-transferable, and does not guarantee
current university status. Fraudulent use or reproduction
of a UW-Baraboo\Sauk
County ID card violates Wisconsin criminal statutes
and will be referred for prosecution.

USE OF YOUR PHOTO ID
| PLEASE NOTE
Your Photo ID card will be valid throughout
your entire academic or employment
career at the University of Wisconsin
Baraboo\Sauk County, or damaged due
to abuse, you will be charged a replacement fee (payment by cash or check only).
The replacement fee is subject to change without
notice.
NEW or RE-ENTRY STUDENTS
If you are issued a card prior to the
start of the term/semester in which you have
enrolled, you may not be entitled to use the
card until the first class day of that term/semester.
Please check with the specific facility listed
below if you have questions.
REMINDER: The 11th digit of your campus ID
number is the issue code (lower left corner).

1. LIBRARY:
(files update every Wednesday night)
- Your UW Baraboo\Sauk
County ID card must be presented to borrow materials
from campus libraries.
- Your ID card is not transferable to another user.
- If you lose your card, you are responsible for notifying
the UW Baraboo\Sauk
County Library
of the loss. You will be held financially responsible for
materials borrowed with your ID prior to you reporting its
loss.
If you wish to check books out from the library
prior to the next update the staff at the library will need
to be informed of your new card number. If you received a
replacement card, your issue code number will have changed
and staff still will need to update their database. You can
update your ID status in person, by calling the Library at
608-356-8351 ext
251
or by email, BRBLibrarian@uwc.edu.
2. RECREATIONAL FACILITIES: (files update nightly)
If you plan to use recreational facilities prior to their
update, let the staff know you have a new card. The date of
issue is printed below your picture.
3. Wiscard
ACCOUNTS: (updates automatically on every Tuesday and
Thursday morning)
If you have a Wiscard account,
you can stop by the
Business Office and update your card information. If
you lose your card, you are responsible for its use until
it has been reported as lost or stolen to the Business
Office 608-355-5210
They will need the following information:
Name, Social Security number, new 11-digit ID card number
and Lot number. You can also fax this information to 608-355-5253
- Attn: Lois. Your
new card will not activate the gate until this has been done.
4. WiscWorld:
Only new accounts
At this point your account is eligible but not active. You
can activate your account by accessing the WiscWorld web page
at www.doit.wisc.edu/wiscworld

NEW AND REPLACEMENT CARD FEES
A cardholder may be assessed a fee to replace their existing
University ID based on the table outlined below. (1)
Fees are subject to change without notice. Payment by cash
or check only.
| FIRST CARD ISSUED: No Charge
REPLACEMENT: No Charge
- Change in status (ie. Fac/Staff to Student)

- Name change (2)
- Switch from Social Security number to
900 number (2)
- Damaged card from "normal" wear and tear
- picture, name, card number, signature,
or bar code is unreadable (2), (3)
- Cardholder's ID number (not card) has
been reported as being inappropriately used
- copy of police report provided (2)
- Returning student does not have their
current ID and has not been enrolled
for the previous two terms (Withdrawn status
should be viewed as not enrolled.)
REPLACEMENT: Charge
- Lost or Stolen card
- Damaged card from improper care or abuse.

- Returning student does not have their
current ID and has been enrolled
during one of the previous two terms
- Patron wants a new ID even though present
card works, is in good condition, and all
information is current (4)
- The
replacement card fee is
$15.
- The current valid ID card
must be surrendered to receive a replacement
card at no cost, otherwise this will be
considered a lost card transaction.
- Card office staff will determine
if card damage is from normal wear.
- Whenever possible the card
office will confiscate ID cards that are
no longer valid
